 NOTE: Please double check with your insurance company that our services will be covered, before your INITIAL visit.  With ALL insurance companies, the patient is responsible for any deductibles, co-payments, co-insurance and non-covered services.  

If your insurance company is not listed above, we will make arrangements with you to submit any claims to your insurance carrier on your behalf.  

Payment is accepted in the form of cash, check and credit card. 

A 3% credit card and handling fee will be added to ALL credit and debit card transactions.
