We accept most healthcare insurance plans. We accept assignment for Medicare, Railroad Medicare, and most Medicare Advantage plans, as well as Horizon Blue Cross/Blue Shield, CIGNA, United Healthcare, and MAGNACARE. 

For patients that have Aetna, please call our office before your scheduled appointment for verification of coverage.

If your insurance company is not listed above, we will make arrangements with you to submit any claims to your insurance carrier on your behalf. 

*Note: Please double check with your insurance provider prior to your first office visit that services rendered by Dr. Jani will be covered. With all insurance companies, the patient is responsible for all deductibles, co-payments, co-insurance and non-covered services. 

Payment is accepted in the form of cash, check and credit card.

A 3% credit card and handling fee will be added to your total.
